End of Life Planning

Having conversations with family, health care professionals, and close friends are important for planning your end-of-life care. Organizations like PCOA can help you begin those conversations and give you reliable information on filling out your advanced directives. In addition, these plans allow you to have the care and treatment you would want if you no longer can make the decisions on your own. The sub-categories here will assist you in this process. In the future, the Arizona Advance Directive Registry will be hosted and maintained by a healthcare organization called Health Current. As a result, a person’s end-of-life care plans will be accessible by healthcare professionals as part of their electronic health record.

Casa de la Luz is a full-service 24/7 hospice agency, offering in-home routine home hospice care where an individual resides and hospice respite services and general inpatient hospice services in our hospice inpatient unit—Casa Hospice at The Hacienda. Services are provided by an interdisciplinary team of physicians, nurse practitioners, nurses, social workers, chaplains, home health aides, and volunteers. We have a bereavement program offering grief support groups, individual grief counseling, and other special grief support workshops. Our bereavement program is open to loved ones of patients who have been cared for by Casa de la Luz Hospice and other individuals in the community. Casa de la Luz has a dedicated interdisciplinary team of Spanish-speaking staff and volunteers. We also have a Community Based Palliative Care Program licensed by Arizona. Casa de la Luz operates Kanmar Place Hospice Home, a residential hospice home. Adult grief support groups are held on Fridays from 10:00 AM - to 11:30 Am and 12:30 PM to 2:00 PM.

Hi, my name is Jess, and I’m an End of Life Doula. Have you heard of an EOL Doula ( or death doula)? That’s ok; I’m here to educate and empower those near end of life or just doing advanced preparation.
I guide those that are dying through the process. Together we work through issues you may want to be resolved before passing, things you may be afraid to discuss with anyone else and work on lessening the fear of dying. We also can design and work on a legacy project, something you’d like your loved ones to have, to remember you by. We will also plan your vigil ( the atmosphere you want and the rules your loved ones need to honor while you’re passing.) I will be there with you every step of the way to ensure your vigil plan is adhered to and guide you and your loved ones through active dying. After your passing, I will connect with loved ones to ensure they are experiencing a healthy grieving process.
Nobody should die alone and afraid. I am here to help empower you through death. Studies show that those who use an End of Life Doula experience less pain and have an easier death than those who don’t have a doula.
I would love to talk with you to discuss how I can help you in more detail. Remember, you don’t have to go through this alone.
