Going Home with Care is a partnership between Valley Assistance Services and Green Valley/Santa Cruz Hospital. It provides care transitions from hospital to home at discharge with specific diagnoses to help ensure recovery and discharge follow-through. Our VAS RN includes home visits to assess each client's health for care needs, providing clients have needed medications and knowledge of drugs used to reduce readmissions.
