Medical home health is a prescription for therapy a doctor gives to remedy an illness or injury at home. A medically licensed therapist visits the patient's home at a convenient time. The therapist belongs to a medically licensed home health agency that the doctor works with to provide medical care at home.
We provide care for wounds, surgical wounds, and rehabilitation, as well as providing intravenous nutrition therapy, injections, severe illness and unstable health status monitoring, and patient and caregiver education.
Medical home health is used in conjunction with outpatient therapy. Therapy at home can be started first if the patient cannot get to the outpatient therapy center. Also, if outpatient therapy services are exhausted from the insurer's standpoint, medical home health can be used to continue therapy.
