Geriatric Care Management
Geriatric care managers can provide comprehensive assessments and care plans, coordinate in-home care services and community referrals, provide crisis intervention, and act as liaisons to out of town family members. Some care managers are certified private fiduciaries and can provide financial management or be appointed as guardians or conservators. Rates, services, experience, and credentials vary among agencies, so comparisons are appropriate. Pima Council on Aging (PCOA)
description
Case management is one service available to clients enrolled in the Community Services System (CSS). The CSS is a program that provides in-home support for older adults who have challenges with daily living activities. CSS assistance is prioritized for those in the most significant social and economic need.
PCOA case managers conduct in-home assessments to understand individual circumstances and needs, connect with community resources, and determine eligibility for CSS assistance. CSS services may include grocery shopping, personal care (e.g., bathing and dressing), light housekeeping, durable medical equipment, adaptive devices, and an emergency alert system. If CSS services are authorized, case managers regularly arrange for their delivery, monitor quality, and reassess needs.

We assist with discharge planning for a smooth transition between hospitals or facilities and home or assisted living. We arrange all kinds of services for our clients wherever they are and monitor them to ensure quality care. We are the boots on the ground for out-of-town families and support local family members caring for their loved ones. We assist older adults in remaining as independent as possible with a safety net for urgent matters. We're available to assist with temporary snowbird support, hospital discharge support, or move stabilization, or we'll stay with you for as long as you and can help plan for the end of life care. 24-hour on-call for established clients.
