End of Life Planning

Having conversations with family, health care professionals, and close friends are important for planning your end-of-life care. Organizations like PCOA can help you begin those conversations and give you reliable information on filling out your advanced directives. In addition, these plans allow you to have the care and treatment you would want if you no longer can make the decisions on your own. The sub-categories here will assist you in this process. In the future, the Arizona Advance Directive Registry will be hosted and maintained by a healthcare organization called Health Current. As a result, a person’s end-of-life care plans will be accessible by healthcare professionals as part of their electronic health record.

Casa de la Luz is a full-service 24/7 hospice agency, offering in-home routine home hospice care where an individual resides, as well as hospice respite services and general inpatient hospice services in our hospice inpatient unit—Casa Hospice at The Hacienda. Services are provided by an interdisciplinary team of physicians, nurse practitioners, nurses, social workers, chaplains, home health aides, and volunteers. We have a bereavement program offering grief support groups, individual grief counseling, and other special grief support workshops. Our bereavement program is open to loved ones of patients who have been cared for by Casa de la Luz Hospice as well as other individuals in the community. Casa de la Luz has a dedicated interdisciplinary team of Spanish speaking staff and volunteers. We also have a Community Based Palliative Care Program licensed by the state of Arizona. Casa de la Luz operates Kanmar Place Hospice Home, a residential hospice home.

We can provide relief with the reassurance that our professional staff is well experienced and able to provide compassionate care for the client and guidance to the family and caregivers. Physical, social, spiritual, and emotional care are provided by a clinically-directed interdisciplinary team consisting of patients and their families, professionals, and volunteers during the last stages of an illness, dying process & bereavement period.
BEREAVEMENT: Our bereavement program supports the patient and families and the caregivers and extended kin of the patient. Our new caregiver center is open to all community caregivers to relax, unwind, and process their experiences. Once a patient has died, we continue to support their loved ones for over a year with cards, phone calls, letters, and visits.

A weekly bereavement support group for individuals experiencing the loss of a loved one by death. This is a safe place to share thoughts and feelings with others who care and understand the journey of grief. The leader and facilitator, Nathan Gladish, facilitates the group interaction similarly as Janna Excell used to lead the sessions before her death. Call Nathan at Sunrise Chapel (leave a message or call his cell phone) before attending the first time. No set curriculum. New participants are welcome anytime, a community service sponsored by Sunrise Chapel. Group meets on Thursdays from 6:30-8:00 pm in person. No online component is offered at this time.
