End of Life Planning

Having conversations with family, health care professionals, and close friends are important for planning your end-of-life care. Organizations like PCOA can help you begin those conversations and give you reliable information on filling out your advanced directives. In addition, these plans allow you to have the care and treatment you would want if you no longer can make the decisions on your own. The sub-categories here will assist you in this process. In the future, the Arizona Advance Directive Registry will be hosted and maintained by a healthcare organization called Health Current. As a result, a person’s end-of-life care plans will be accessible by healthcare professionals as part of their electronic health record.

Home health, Hospice, and Home Care Services. Providing RN/PT/OT/MSW/ST services to those in need in the greater Tucson and Vail areas. Eden Home Health helps with the follow-up on physician's orders so that you can recover in your home safely with the care you deserve. Eden Hospice provides end-of-life care to the patient and family with an Interdisciplinary Team comprised of an RN, Medical Director, Chaplain, Social Worker, and Certified Nursing Assistant, as well as volunteers and those you may need to help your loved one. Home Care services are provided at cost to the client. This service can give in-home light cleaning, assistance with doctor's appointments and grocery shopping, companionship services, and much more.
