End of Life Planning
Having conversations with family, health care professionals, and close friends are important for planning your end-of-life care. Organizations like PCOA can help you begin those conversations and give you reliable information on filling out your advanced directives. In addition, these plans allow you to have the care and treatment you would want if you no longer can make the decisions on your own. The sub-categories here will assist you in this process. In the future, the Arizona Advance Directive Registry will be hosted and maintained by a healthcare organization called Health Current. As a result, a person’s end-of-life care plans will be accessible by healthcare professionals as part of their electronic health record.
End of Life Education
End of life care education provides information and resources to assist people who are thinking about, planning for, and documenting their end of life care plans (also known as advanced directives). End of life education clarifies medical and legal terms, answers frequently asked questions, and creates a respectful space for considering your end of life options for health care and creating a legacy. Presentations are available in 1 or 2-hour versions, can be provided in English or Spanish, and are customizable to meet the unique needs of your company, place of worship, or community group. Some organizations, like PCOA, can also develop a unique presentation to address specific end of life issues or concerns that are important to you.

End of life doula and coach works primarily with clients regarding conscious aging and life transitions. The creation and facilitation of workshops for organizations to support their specific needs are offered, and the owner is a master facilitator and a certified coach. Individual sessions are offered on most aspects of death and dying and planning and facilitating living funeral ceremonies.
Consulting costs vary and start at $150 per hour. Project prices are also quoted. Coaching is offered at $150 per hour, and package pricing is available.

Thoughtful Life Conversations is an Arizona Hospital and Healthcare Association program that promotes patient choice, values, and autonomy through advanced care planning and advanced provider competencies for honest conversations and care. Strengthens public policies and payment systems to serve those with advanced illness better.
Thoughtful Life Conversations also includes the Arizona Coalition to Transform Serious Illness Care to develop new care models that are value-driven and allow Arizonans to receive the care they want in the setting they chose.

El Rio Healthcare currently offers the El Rio community Advance Care Planning (ACP) for their end-of-life medical decision-making needs with an RN specializing in ACP. We use the AzHHA Advance Directive Short Forms: Arizona Medical Living Will and the Arizona Healthcare Power of Attorney with Optional Mental Health Authority.

El Rio receives funding from many health insurers, patient co-pays from our sliding fee scale, government grants, AHCCCS, and private donations to El Rio Health Center Foundation. No one is denied services solely based on the inability to pay.

Advance care planning is a way to help ensure that your values in life and wishes for care will be honored through your life if you are ever unable to speak for yourself. Interfaith Community Services (ICS) offers FREE workshops to help people begin, continue or revise their advance care planning.

Life and Work Connections provides free, confidential wellness assistance and addresses remote caregiving for all benefits-eligible staff and faculty employees of the University of Arizona. . Dependent care services are also available to students.
The office offers support for dependent care, mental wellness, physical health and nutrition, and financial literacy concerns. Services include individual consultations, group workshops, training sessions, and articles and videos on a wide spectrum of health and well-being topics.
The dependent care advisor at Life and Work Connections supports faculty, staff members, and students who provide care for adults and elders. The advisor helps caregivers develop individualized plans of care unique to their comprehensive challenges. The dependent care advisor researches and navigates pertinent resources, such as location- or diagnosis-specific aid, to guide caregivers to the support that meets their particular needs.
The dependent care advisor’s services include one-on-one meetings and workshop presentations on caregiving conversations to help employees and their loved ones ensure that their quality-of-life wishes are upheld and necessary financial, legal, and medical documentation is correct.
